Review Within 24 Hours
The clinical coordination team reviews every referral within 24 hours of submission.
Eligibility Confirmation
The coordination team confirms program eligibility through the Unity Access Care Services intake and assessment process. The referring party is notified of the outcome directly.
IRP Development
An Individual Rehabilitation Plan is developed in coordination with the participant and their clinical team prior to program placement. The referring party is kept informed at this stage.
Program Placement
The participant is placed in the appropriate Independence Division programs based on their IRP goals, assessed needs, and program availability.
Ongoing Coordination
The center maintains active coordination with the referring team throughout the participant's program engagement, providing progress updates.
