Clinical Rehabilitation Programs

The Independence Division delivers structured, goal-directed skill-building programs for individuals enrolled in the Unity Access Care Services Psychiatric Rehabilitation Program. All programs are developed and delivered in compliance with applicable regulatory standards — tied to each participant's Individual Rehabilitation Plan, administered by qualified staff, and supported by licensed clinical supervision throughout.

Before Referring — What Every Referral Requires.

Independence Division programs are clinical rehabilitation services. Enrollment is structured and requires the following to be confirmed prior to program placement:

An active PRP referral from a licensed or authorized referral source

A confirmed mental health diagnosis consistent with PRP eligibility criteria

Verified program eligibility determined through the Unity Access Care Services intake and assessment process

All client information received through the referral process is handled in strict accordance with applicable privacy and confidentiality standards. Personal health information is never collected through unsecured channels.

Independence Division participant

Eight Structured Programs. One Coordinated Rehabilitation Framework.

Every Independence Division program is built around each participant's Individual Rehabilitation Plan (IRP). Progress is consistently documented, reviewed monthly, and coordinated with the participant's clinical team throughout their time in the program.

Daily Living Skills Lab

Activities of Daily Living | Independent Living Skills

This program helps participants build the everyday routines required for independent living. Core areas include personal hygiene, meal planning, basic nutrition, home organization, and medication management. Sessions are practical and hands-on, with participants actively practicing skills while receiving guided support. Progress is tracked against each individual's rehabilitation goals.

CommunicateWell

Social & Communication Skills Training

CommunicateWell focuses on strengthening how participants interact with others in everyday life. The program covers conversation skills, conflict resolution, relationship building, and appropriate behavior in social and community settings. Each session is structured to build confidence through practice, with measurable progress tied directly to IRP goals.

WorkReady

Vocational & Employment Readiness Training

WorkReady prepares participants for the realities of entering and maintaining employment. The program covers resume development, interview preparation, workplace expectations, time management, and following instructions in a professional environment. It is designed for individuals managing employment challenges alongside ongoing mental health recovery, providing both structure and practical guidance.

MoneyWise

Financial Literacy & Money Management

MoneyWise builds practical financial skills that support long-term independence. Participants learn how to create and manage budgets, track expenses, handle bill payments, and recognize risks such as financial exploitation. Sessions focus on real-world application, with progress documented against each participant's financial goals.

CommunityConnect

Community Integration Skills Training

CommunityConnect takes learning beyond the classroom by helping participants build confidence in real-world environments. Participants practice navigating public transportation, accessing community resources, shopping for essentials, and managing appointments independently. Each session includes guided field experience followed by a structured debrief to reinforce learning and track progress.

MindBalance

Coping & Mental Health Skills Training

MindBalance focuses on strengthening each participant's ability to manage stress, emotions, and everyday challenges. The program covers emotional regulation, identifying triggers, managing anxiety and anger, and using grounding and breathing techniques. Participants learn practical strategies they can apply in daily life, with consistent documentation to support continuity of care.

CrisisReady

Crisis Prevention & Stabilization Support

CrisisReady helps participants recognize early warning signs and develop clear, personalized plans to prevent and manage crises. The program also teaches practical de-escalation techniques that can be used in high-stress situations. All work is closely aligned with the participant's clinical team to ensure consistency with their overall care plan.

Family Circle

Family Education & Caregiver Support

Family Circle supports caregivers and family members by giving them the tools to better understand and reinforce the participant's progress. The program covers rehabilitation principles, ways to strengthen the home environment, and practical strategies for ongoing support. It ensures that progress made in sessions continues outside the center.

Every Program. Every Participant. One Plan.

No program operates in isolation. Each participant's experience is guided by a structured Individual Rehabilitation Plan (IRP) developed in coordination with their clinical team.

Progress is documented across all programs, with monthly reviews used to evaluate outcomes and adjust program placement or intensity as needed. The center maintains consistent communication with referring providers and prescribers throughout the process.

This is not a referral-and-release model. The center remains an active partner from enrollment through completion.

Every referral is reviewed within 24 hours, with urgent cases prioritized.

Eligibility Confirmation

Eligibility is confirmed through the Unity Access Care Services intake and assessment process, and the referring party is updated.

IRP Development

An Individual Rehabilitation Plan is developed in collaboration with the participant and their clinical team.

Program Placement

Participants are placed into programs based on their goals, assessed needs, and availability.

Ongoing Coordination

The center continues to coordinate with the referring team, providing updates and communicating any clinical concerns.

Contact Clinical Team

Additional support services—including housing coordination, employment support, and case management—are available through Unity Access Care Services.

Open Enrollment Community Programs

The Public Programs Division offers practical, professionally structured training programs open to any adult in the community. There are no referrals, no clinical requirements, and no barriers to entry—just real-world skills training delivered in a supportive and professional environment.

Programs focus on employment readiness, financial literacy, daily living skills, wellness, and vocational development.
